This dissertation aimed to compare two managerial models used by the Health State Secretariat of Health in the State of São Paulo in the management of hospitals: there are hospitals directly managed by the State Government and other managed by not for profit organizations, called Social Health Organizations (OSS). The OSS were analyzed as special case in the health State reform. Both models were compared according their manager’s views regarding managerial practices in four areas: human resources, budgeting & finances, out-sourcing and supply management. Managers of five directly managed and five OSS were interviewed for a qualitative research. The comparison showed an advantage in the OSS model, except for the out-sourcing area. Nevertheless, the OSS model does not have appropriate answers neither for accountability, nor for social control issues. Public administration must be reformed to improve its managerial capacity.
