The social capital within Community Health Workers (CHW's) is essential for the performance and their functions., It is through the community's trust in the that their actions are developed seen in a professional manner, through home visits. Therefore, for public management, the agent's productivity is measured by the number of monthly visits in the community, which is their main work activity. However factors, such as social interactivity, information exchange, cooperation, knowledge, can not be measured in the Family Health Strategy's productivity reports. Thus, the present study had as objective to analyze the influences of social capital, of the Community Health Workers on the performance of the Family Health Strategy (FHS) in Brazilian cities. The theoretical basis used was the social capital approach, due to it affirming that social capital influences: improving health and promoting healthy behavior habits, which increase access to health services and facilitates the development of processes. In order to understand this phenomenon, three municipalities were selected in different regions of Brazil, according to Geiger's geo-economic division (1964), homogeneous municipalities (CALVO et al., 2016) regarding population size and coverage of health services. In the data collection, three techniques were used: semi-open questionnaire with name generator (n = 333), semi-structured interviews with FHS directors (n = 3), and CHWs productivity reports in 2017. For the analysis of this data a multiple linear regression model was proposed to identify the relation of social capital with productivity indicator. A content analysis for the interviews and open questionnaire questions. Finally, Social Network Analysis (SNA) with the use of cohesion and trust measures, with centralization, density, intermediation, proximity and E-index metrics. It was verified that the social capital is part of the motivation for the work, acting in the community, exchange of information and social interactivity. The number of visits (CHW productivity) is a limited indicator, with low importance in the explanation of social capital. Factors such as resoluteness, lack of recognition, lack of capacity building, infrastructure and resources, directly influence the performance of CHW's. The relationships with doctors and nurses, constituting a social capital linking type, provides greater diversification and dissemination of knowledge and information. The CHW exerts influence in the FHS, not only through its home visits, but in the relationship with other professionals, acting as a bridge actor, by relating to actors with low connections to the community. And by occupying central positions in their relationship networks.
